    (e.g., 01, 02, 03, or 04)

  3. Item Cost Type
    Select from the following:

    • "Item"
    • "Item Plus Freight"
  4. Contact Name
    (e.g., Melvin Smalls, Contact at your company)

  5. Contact Phone
    (e.g., 704-555-1092, Contact phone)

  6. Contact Email
    (e.g., orders@furniturecompany.com, Contact email)

  7. All Ship To Addresses
    (e.g., 1690 University Rd. Pl, Charlotte, NC 28213)

  8. Store Name
    (e.g., Furniture Store)

  9. PO Option
    Select from the following:
